Know the training and education of the dental professionals taking care of you
Arrange to see your dentist each time you come into our office for treatment, subject to Wisconsin state laws
Adequate time to have your questions answered and concerns addressed regarding your condition and treatment
An explanation of the purpose of the treatment, probable results, risks, and alternatives, before you give consent to a treatment plan
Be informed of healthcare needs that are continuing
Know the expected cost of treatment in advance
Reasonable arrangements for emergency treatment and dental care
Receive confidential, respectful, and considerate treatment by your dental team and dentist in Milwaukee
Expect your dental team members to use proper sterilization and infection controls/procedures
Ask about the availability of dispute-mediation processes regarding your treatment

You have a responsibility to:

Ensure that the financial obligations regarding the healthcare you received are fulfilled
Follow recommendations for regular, at-home oral healthcare
Be available for dental treatment when given reasonable notice
Keep your appointments that you scheduled
Face the consequences you experience as a result of declining treatment of not adhering to the treatment plan that was agreed upon

Ask questions about your condition and treatment, as well as actively participate in your healthcare decisions
Report alterations in your medical status as well as provide feedback about your expectations and needs
Provide complete and honest information regarding your current health status and medical history, to the best of your ability
